Early Dismissal - If you have to take your child out of school early, please call in advance, or send your child in with a note for the front office. You MUST present your driver’s license when you arrive. A work ID is not valid.

Great job to all students and staff for MCAP and ELA testing. We are proud of your hard work and perseverance. As a reminder sixth graders will take MISA (science) next week.
As we continue our Be the Good - See the Good focus this year, we ask that each family discuss the serious implications of several social media “challenges” students may be aware of at this time. These include things like destroying school property, stealing from school and/ or striking or causing harm to staff and peers. As you are aware, these types of behaviors will not be tolerated and have implications from the code of conduct. We are grateful for the hard work, kindness and positive decision making that we have been seeing from our students this year and want to ensure that this continues.

MISA Testing - Testing will be on Wednesday, October 6 for 6th grade only, during Periods 1 and Falcon Time. Testing is expected to conclude by 9:00 am.Students MUST bring and use an HCPSS Chromebook AND charger for testing. They will not be able to use a personal laptop. Students who have wired headphones are strongly encouraged to bring them to all testing sessions. (Bluetooth/wireless headphones cannot be used for testing).

FQMS Flu Clinic - Flu vaccinations are coming to your child’s school on October 27, 2021. They will be given at NO CHARGE TO YOU. Now, more than ever, it is important to protect your family through vaccinations.
If you would like your child to be vaccinated, please register BEFORE Friday, October 8:
Fill out the consent form online here.
Type your child’s school name in Search by Name then press the blue Search button.
Click on the Sign Up for Flu Vaccinations button for your child’s school.

Fill out the paper consent form attached:
Read the Vaccine Information Statement (VIS) and talk to your health care provider or the school nurse about any questions you have. VIS: https://www.immunize.org/vis/flu_inactive.pdf
Fill out every section of the attached Consent Form.
Use your insurance card to fill out your insurance information accurately. Your insurance company will be billed. There is no co-pay or deductible. You will NOT be charged by us or your insurance company.
You may give permission for your child to be vaccinated using (1) an online form here. Or (2) the hard copy form attached to this message.
If you do not complete the online consent form, please return the hard copy of the consent to your child's nurse by October 8, 2021.
